Upper Crossed Syndrome vs Cervical Radiculopathy: Neck Pain and Posture

Introduction

Neck pain with arm symptoms is common. The challenge is distinguishing between a structural nerve root problem (cervical radiculopathy) and a postural muscle imbalance pattern (upper crossed syndrome) that can produce remarkably similar symptoms.

Upper Crossed Syndrome

Described by Vladimír Janda, this postural pattern involves:

Tight/overactive muscles: Upper trapezius, levator scapulae, suboccipitals, pectoralis major and minor, SCM

Weak/inhibited muscles: Deep cervical flexors, lower trapezius, serratus anterior, rhomboids

Result: Forward head posture, rounded shoulders, cervical hyperextension. This posture narrows the intervertebral foramina and can compress cervical nerve roots dynamically — producing pseudo-radicular symptoms.

Symptoms: Neck pain, headaches, upper back pain, shoulder tension. May include arm heaviness, diffuse tingling, and grip fatigue. Symptoms are typically bilateral, posture-dependent, and relieved by movement or posture correction.

Cervical Radiculopathy

Cause: Structural compression of a nerve root — disc herniation, osteophyte, foraminal stenosis.

Symptoms: Unilateral arm pain in a dermatomal distribution. Specific weakness (myotomal). Reflex changes. Symptoms may be constant or worsened by neck positions that further narrow the foramen.

Clinical Pearl

The deep cervical flexor endurance test is valuable: patient supine, tuck chin, lift head 2cm off the bed. Hold time under 20 seconds suggests deep flexor weakness — a hallmark of upper crossed syndrome. This simple test shifts your clinical thinking toward postural rehabilitation rather than imaging.

Summary

Bilateral, posture-dependent, no neurological signs = upper crossed syndrome. Unilateral, dermatomal, with weakness or reflex changes = cervical radiculopathy. The two can coexist — treat the posture, but don't miss the radiculopathy.
